Our services
Maple Crest provides non-medical wellness visits and related family communication services. We are not a medical provider, and nothing in our visits or platform should be treated as medical advice, diagnosis, or treatment.
Consultations and booking
Requesting a consultation through our website does not create a service agreement or guarantee a scheduled visit. A member of our team will follow up to confirm details and, where both parties agree, establish a care plan and service agreement.
The Family Portal
Access to the Family Portal is provided to authorized family members of active clients. You're responsible for keeping your account credentials secure and for any activity under your account.
Emergencies
Our care team follows a structured emergency response process, including contacting emergency services when appropriate. Nothing about our service replaces 911 or direct emergency medical care in a genuine emergency.
Changes to these terms
We may update these terms from time to time. Continued use of our services after an update constitutes acceptance of the revised terms.
